Repair Form
Please read the following information carefully before submitting your pen(s) for repair:
 -All fields marked with a red asterisk on the repair form below must be completed
 -You must enter a physical shipping address and not a PO Box  (we are unable to ship to a PO Box)
 -Once you have completed the repair form below and clicked ‘submit’ at the top, you will be taken to our secure payment site.  You must enter your credit card information. Please know your card will not be charged until the repair is completed!
 -You will then be redirected to a confirmation screen which will include a confirmation number at the top (e.g. SO12345).
 -Once you complete the above,  please print the confirmation page and send with your pen(s) for repair. Your confirmation number must also be written on the outside of your package.
 *WE CANNOT REPAIR YOUR PEN WITHOUT YOUR CONFIRMATION EMAIL
